ACKNOWLEDGEMENT OF K & D ELECTRIC
ISSUED MATERIALS & EQUIPMENT

I understand that “items” (i.e. uniforms, equipment, etc.) are to be issued to me for use as an employee of K & D Electric and that upon the end of my employment these items must be returned in good condition, routine wear and tear are expected. If, during the course of my employment, any of the issued items become missing or damaged, I understand that I am to notify K & D Electric as soon as possible of the damage or loss and the applicable circumstances, so that replacement items may be obtained. Due to safety concerns, it is imperative that only K & D Electric approved replacements be obtained.

I hereby authorize K & D Electric to deduct from my paycheck, including my final paycheck, the replacement cost for any lost or damaged items that must be replaced due to my negligence. If the items are lost or damaged during K & D Electric operations, I understand I am not liable. I understand, that if at the time of separation, I attempt to turn in damaged equipment or report equipment missing or lost, and the equipment had not been previously reported as damaged, missing or lost, that it will be conclusively determined that the damage or loss was not during K & D Electric’s operations.
